What to Expect from Home Care Physical Therapy?
- A licensed physical therapist will travel to your home and evaluate your condition or injury. A highly individualized program will be designed to meet your specific rehabilitative needs.
- The therapeutic methods during the course of treatment are established to achieve the goals set by you and the physical therapist. Goals are based on improving the quality of life and attaining independence.
- An assessment of the home including bathrooms, bedrooms, doorways, etc. is performed and suggestions to improve safety and mobility are made to promote independence.
- An assessment of the patient’s functional activities relating to daily living will be made and recommendations will be provided for any appropriate assistive devices. This will help the patient strive for independence.
- A home exercise program will be established and continually updated to accommodate progression during the treatment plan.
- Physical therapists travel with mobile state of the art equipment to meet the therapeutic needs of each individual patient.
In Order to Receive Home Care Physical Therapy the Following are Necessary:
- Patient is unable to leave home unattended
- A doctor’s prescription specifying home care
- If the insurance company requires a referral then a referral is necessary
